The section below contains information about one of the tabs in the Family Planning (FP) Report.

Reminder: For each visit, a KDHE Program Visit Form – Adult and a Service Form must be completed with matching Date of Activity and the correct program selection to ensure that all clients/visits are counted in reports. If there is no KDHE Program Visit Form – Adult with Family Planning selected for the Program that matches the Date of Activity for a client, their information will not be included in this report.

Data Source

Unduplicated Family Planning Clients Tested for Chlamydia by Age Group and Sex is calculated from ‘Date of Birth’ and ‘Gender’ on the Caregiver Adult Profile and ‘Chlamydia Test’ selected as the response to ‘Program Services’ for clients who have a FP Service Form during the date range. Client age is calculated based on their most recent visit during the date range.

Troubleshooting Tip

This is an unduplicated count of clients with ‘Chlamydia Test’ selected as a Program Service on their most recent Family Planning Service form during the selected date range.

This chart can be filtered by gender by selecting the desired caregiver gender from the filter on the right side of the screen.

Number of Chlamydia, Gonorrhea, Syphilis, and HIV Tests is calculated from ‘Gender’ on the Caregiver Adult Profile and ‘Chlamydia Test’, ‘Gonorrhea Test’, ‘Syphilis Test’, and/or ‘HIV Test’ selected as the response to ‘Program Services’ on the FP Service Form.

Troubleshooting Tip

This is a count of all HIV tests results entered on Family Planning Service forms during the date range.  Note that the total for this chart may not match the number of HIV tests provided. The count of HIV tests provided comes from ‘HIV Test’ being selected in the Program Service dropdown, while information from this chart comes from the ‘HIV Test Results’ field.  Organizations with a discrepancy may want to review their Family Planning Service forms and update any missing data, either in the Program Service selections or HIV Test results field to ensure high quality of data in this report.
