18- MCH Referral Report

The section below contains information about one of the tabs in the Maternal & Child Health (MCH) Report.

Reminder: For each visit a KDHE Program Visit Form and a Service Form must be completed with matching Date of Activity and the correct program selection to ensure that all clients/visits are counted in reports.
Note: Instructions for accessing underlying client level data and downloading PDF versions of reports are available in the DAISEY TA Brief “Using Reports for Quality Assurance”.

18- MCH Referral Report

Data Source

Referrals Made is calculated from ‘Maternal Child Health’ selected as ‘Program’ and ‘Yes’ response to each type of referral service on the KDHE Program Referral Form within the date range.

Troubleshooting Tip

This report reflects all referrals made during the selected date range, so there may be multiple referrals per client and per visit.

Data Source

Referrals Completed is calculated from ‘Maternal Child Health’ selected as ‘Program’ and ‘Yes’ response to each type of referral and any of the ‘Yes’ selections in the corresponding ‘Referral completed?’ field on the KDHE Program Referral Form during the date range.
