13- Healthy Start Home Visits: Setting and Provider Information

The section below contains information about one of the tabs in the Maternal & Child Health (MCH) Report.

Reminder: For each visit a KDHE Program Visit Form and a Service Form must be completed with matching Date of Activity and the correct program selection to ensure that all clients/visits are counted in reports.

Data Source

Number of Healthy Start Home Visits by Setting is calculated from ‘Is this a Healthy Start Service?’ and ‘Setting of Visit’ selected on the MCH Service Form during the date range.

  • Setting of Visit

Troubleshooting Tip

Note that this total represents the total number of visits, so clients may be duplicated if they had more than one visit during the selected date range.

Troubleshooting Tip

This report represents the total number of visits, so clients may be duplicated if they had more than one visit during the selected date range.
