The section below contains information about one of the tabs in the Teen Pregnancy Targeted Case Management (TPTCM) Service Report.

Reminder: For each visit a KDHE Program Visit Form and a Service Form must be completed with matching Date of Activity and the correct program selection to ensure that all clients/visits are counted in reports.

Data Source

Life Births by Gestational Age is calculated from ‘Gestational age of infant at birth (in weeks)’ on the TPTCM Service Form with an infant’s date of birth during the date range.

Question:

Gestational age of infant at birth (in weeks)
Date of infant’s birth

Troubleshooting Tip

Only types of birth outcomes and gestational ages that are represented in the data will be reflected on the charts and/or tables. If an outcome or gestational age is not represented in the chart/table, it is not in the data set.
